As a Patient, You Have the Right To:
- Be treated with courtesy, respect, and consideration.
- Receive care in a safe setting and have your privacy protected.
- Confidentiality of your medical record and protected health information.
- Access information contained in your medical record within a reasonable time.
- A prompt and reasonable response to questions and concerns.
- Know the identity of the physician, provider, and individuals involved in your care.
- Receive information about available patient support services.
- Receive information about hospital rules and regulations.
- Refuse treatment to the extent permitted by law.
- Receive information regarding financial resources available for care.
- Receive a reasonable estimate of charges when practical.
- Receive a clear and understandable itemized bill.
- Impartial access to treatment and accommodations.
- Receive emergency screening and stabilizing treatment as required by law.
- Be informed of experimental research and consent or refuse participation.
- Receive information about diagnosis, treatment options, risks, and prognosis.
- Have pain assessed and managed appropriately.
- Receive visitors of your choosing subject to reasonable restrictions.
- Have a support person or representative notified of admission when requested.
- Be free from restraints or seclusion unless necessary for safety.
- Voice complaints or grievances without fear of retaliation.
As a Patient, You Have the Responsibility To:
- Provide accurate and complete health information.
- Report unexpected changes in your condition.
- Ask questions when you do not understand your care.
- Follow the recommended treatment plan.
- Keep appointments and notify staff when unable to attend.
- Accept responsibility if refusing treatment or instructions.
- Fulfill financial obligations for healthcare services.
- Follow hospital rules and regulations.
- Maintain respectful behavior toward others.
- Respect the rights, privacy, and property of others.
