West Ascension Parish Hospital provides patients with access to their medical records in accordance with applicable laws and regulations.
To request medical records, please complete the appropriate authorization form and submit it to our Health Information Management department.
Requests must include valid identification and sufficient information to locate the requested records.

• Submit the form by email, mail, or in person
• Allow processing time for review and fulfillment
Processing Time
Please allow up to 7–10 business days for processing. Additional time may be required for large or complex requests.
